Gazebo Park Permit Application & Release of Liability

The resident submitting this form (the undersigned) hereby requests permission to use the Gazebo Park in Section 3 of the Village of Chevy Chase at the date and time indicated. The online submission of this form is accepted as the e-signature of the applicant.

In consideration of the permission to use the Gazebo Park granted by Section 3 of the Village of Chevy Chase (the Village), the undersigned does hereby for myself, and my successors and assigns, agree to release, discharge, indemnify and hold harmless all claims, costs, causes of action, damages, attorneys fees or liabilities of any nature whatsoever, for injuries to any person or persons and/or their property, that may be sustained in and about the Gazebo Park in connection with the use thereof by my guests, my invitees or me, regardless of whether said injuries are due to negligence or any other fault.

The applicant hereby certifies that the use of said Gazebo Park is completely voluntary, and that by the use of said Gazebo Park neither I nor any of my guests or invitees shall be considered, in any way, the employee, servant or agent of the Village.

In addition to this permit, an Alcoholic Beverage Permit may be required if the applicant plans to serve alcoholic beverages.

In Witness Whereof, the applicant hereby represents and certifies that he/she/they is twenty-one (21) years of age, or older, and that if this Application is being made on behalf of an organization, group or family, that the applicant is authorized to sign this Application on behalf of said organization, group or family.

Notes -  Generally speaking, use of the Gazebo Park is limited to 5 hours. "Park Closed" signs (supplied by the Village Manager) must be posted on both entrances with masking tape at least 24 hours in advance of the event. Submission of this form does not imply automatic approval.  You will be notified by email.  You will also receive a copy of this application via email from "Section 3".
